The Right Medicine
To make drugs affordable, price transparency must cover the entire supply chain
TOI Editorials
Supreme Court, hearing a PIL on the huge gap between what retailers pay for medicines and consumables, and what customers end up paying, wants to know from govt why prices of all medicines, devices, and consumables cannot be capped, given the disparity even for the most basic antibiotic. This comes on the heels of GOI capping the price of certain cancer drugs, a required measure. Additionally, after reports of profiteering by hospitals via exorbitant markups on medicines, Karnataka pricing regulator, this month, directed that hospitals must include both base price (what medicines cost them) and MRP, on all bills for medicines, medical devices and consumables across hospitals. Step back, to recall that in 2018, GOI told Parliament that in a single year, 3.8cr people fell below poverty line due to spending on medicines. And even today, pricing of medicines/devices/consumables is addressed in fits and starts, in a piecemeal manner. The national pricing regulator has capped prices of stents, knee implants, and some essential and cancer drugs. But there hasn’t been a whole-of-trade approach.
SC exploring the price capping of all unpatented drugs is timely. As is Karnataka’s move. But as industry bodies point out, such measures will have little impact if the price-to-hospital is already artificially inflated. A capping of the mark-up at the point of sale to patients, can hardly fully protect the patient. Hospitals purchase consumables with high MRP and low price-to-distributor. The only way out is to devise an all-India standardised template to track the trade/profit margin from the very beginning of the commercial process, when the drug or device enters the supply chain. Some experts have argued that the first point of sale is where GST is first applied – pharma company to distributor, or for imports, the landing import cost. This is the approach central drug pricing regulator NPPA must take. While Centre has reiterated that enforcement of rates is state responsibility, there are some concerns GOI must address – like medical manufacturers worrying about the skew of a higher GST for inputs, but lower rate for finished products. SC has its finger on the people’s pulse. Transparency in the process of costs and trade margins, across the supply chain, can ensure fairness to patients and industry, and help monitor for profiteering, without disrupting supply.

How To Defend The Rupee
RBI’s in action mode shoring up the currency. But not all short-term moves are costless. Best option? Small, unglamorous reforms that can revive dollar inflows
Sugata Ghosh
A central bank striving to prop up currency is like a police force trying to fix the mafiosi. Beyond a point, it’s an experiment. No monetary authority can treat a structural malady, just as law enforcers can’t cure a social ill. Nonetheless, all the king’s men and horses in Mint Street are battling a slippery slope to back the rupee.
Amid foreign investors pulling out from the stock market, creeping inflation, and nasty geopolitical choke points, they are using blunt, unusual weaponry, to pre-empt a drift of the dollar-rupee rate towards the optically damaging number, 100.
In Mumbai’s money market, the weekend began with RBI taking aggressive steps to clamp down on dollar demand. This is not just demand from punters betting against the rupee. But even from importers, who book dollars in advance to guard against a falling rupee.
About 48 hours later, these measures were extended to ‘offshore’ forward dollar purchase deals, cut by banks in GIFT City, which under foreign exchange laws is considered an overseas territory.
This spate of actions had a singular aim: to prune dollar demand and arrest a further slide in the rupee, albeit at a price. All this happened a week after RBI hiked the interest rate – a policy decision that’s often linked to supporting currency, besides taming inflation – and six months after a drastic cap on banks’ net open position, which captures how much a bank is gambling or is exposed to exchange rate fluctuations.
The last few days’ announcements, coming in the early hours of Saturday and late Monday evening – probably, to prevent local traders from quickly taking positions – drove bankers into a huddle. As they tried to figure out how to implement the regulations, which kicked in once the markets opened on Monday. The odd timings, the nature of new rules, and the confusion that followed, betrayed an uncommon sense of urgency. What was the trigger?
New Delhi may well be running out of patience. The huge mobilisation of foreign currency deposits from the diaspora didn’t solve the problem. Scrapping tax on profits of foreign portfolio investors (FPIs) from govt bond trades didn’t help. So, what now? At some point, New Delhi may have asked for quick results.
However, none of RBI’s recent measures are long-term solutions. A regulator, like its peers at different points, can come down heavily on speculators. But, raising the cost of hedging foreign exchange risk for bona fide importers, is a skewed regulation to rein in currency.
Under the new rule, banks must park 20% of the amount – for instance, $2mn, when an importer buys $10mn, which would be delivered later, when it pays an overseas seller. This makes forward covers more expensive. Why? Since funds parked with RBI fetch nothing, banks would recover the cost from corporate clients that import.
Such measures often lose efficacy as markets have a habit of testing regulators. In 2015, after China tried a similar 20% risk reserve, the fall in yuan wasn’t reversed, though it became slower. Even the rupee surge, post RBI’s curbs in March 2026, fizzled out in weeks.
Will RBI take more steps if the rupee continues to play truant? Well, there’s a lot in a regulator’s arsenal – forcing exporters to sell their dollars early, restricting outflows by companies setting up subsidiaries and family offices abroad, and by wealthy Indians buying stocks in Nasdaq, and homes in Dubai. But, such measures could put off not only the business elite, but even foreign investors, who could construe them as regressive capital controls.
A more serious, less discussed issue that’s come to the fore is whether India would tread the path, something it has rarely done, of raising interest rates to defend the rupee – in the way emerging economies like Indonesia have.
At a time Indian business houses are making large overseas direct investments, global risk capital is moving into AI ventures, overseas private equity houses and venture capitalists are booking profits to transfer funds, and MNCs like large Korean companies are listing here to take the money out, should India consider giving rate shocks to cushion the rupee?
There’s no easy answer here, as hikes in RBI’s benchmark rate would immediately push up rates on home, auto, and other consumer loans.
Suddenly, we are confronting some home truths, which the volatile rupee and an active RBI have brought to the fore. Low-hanging fruits are gone. The age-old macro arithmetic of filling the trade deficit with surplus capital inflows is under strain. For long, our balance of payments has been in deficit. And, there are fiscal challenges in slashing capital gains tax on stocks.
Only a long, tortuous road with small, unglamorous reforms that don’t make headlines, but require the cooperation of state govts, lies ahead. Perhaps, this is the only road that can lead to ‘ease of doing business’, a slow revival of foreign direct inflows, and persuading domestic business houses to take long bets on India, rather than shifting capital overseas. It’s an arduous path, when Centre-state relations are tense. Yet, it’s the only road left. Else, RBI would only tackle the symptoms, not the ailment. But, for how long?

To Make India’s Demography A Dividend…
Govts, industry, educators must deliver ASAP on six changes, from getting more women at work to apprenticeship programmes to bigger incentives for small firms
Paul Alapat, [ The writer is an angel investor ]
Nations, like individuals, occasionally encounter transformational opportunities that demand decisive action. Demographic dividend – a period when a country’s working-age population expands while the proportion of young and elderly dependents shrinks to historic lows – is one such. Properly leveraged, it can dramatically boost a nation’s economic trajectory, raising productivity, increasing savings and investment capacity, strengthening tax revenues, and easing pension burdens. However, this window opens only rarely in a nation’s history, making its utilisation critical.
India’s demographic dividend began in 2005, and is projected to last till 2055, with the peak expected around 2036. We’re near the midpoint of this favourable period, and roughly a decade remains before the dependency ratio starts climbing again. This raises two urgent questions: does favourable demographics automatically translate into economic gains; if not, is India capitalising on this opportunity?
Not a sufficient condition | A growing working-age population creates only the potential for accelerated growth – it does not guarantee it. In Africa, countries like Nigeria, Chad, Niger, Democratic Republic of Congo, Central African Republic, Mali, and Angola, experienced favourable demographic shifts, but failed to convert them into economic prosperity.
Nigeria is a cautionary tale. While East Asian nations and China successfully harnessed their demographic dividends starting in the 1980s, Nigeria couldn’t because 1) its fertility rate declined too slowly, 2) the economy remained overly dependent on oil without diversifying into job-creating sectors, 3) the education system produced graduates lacking employable skills, and 4) governance failures, combined with infra deficits and regional security problems, created additional obstacles.
Closer home, Sri Lanka’s demographic advantage peaked around 2010, but benefits were largely negated by civil war, Covid, and the severe economic crisis of 2022.
Where India stands | Among major South Asian economies, both India and Bangladesh currently have expanding working-age populations. However, there’s a growing consensus that India has not fully capitalised on this demographic opportunity yet.
GDP growth since 2005 has been robust. Falling dependency ratios account for nearly 20% of per capita GDP growth since the 1980s, and the expanding workforce has supported growth in IT services and manufacturing. But job creation is the core problem.
India needs to generate employment to keep pace with its expanding working-age population (15-64 years). But, in sharp contrast to China and East Asia, where export-oriented manufacturing booms readily absorbed surging worker populations, India’s manufacturing sector continues to underperform despite govt incentives and policies.
There are several reasons for this: difficulties in conducting business, inadequate infra (particularly unreliable power supply), and an education system – especially govt schools – that struggles to produce employable graduates. Additionally, India’s female labour force participation ranks among the lowest globally, representing a significant underutilised resource. The large informal employment sector further limits productivity and economic value addition.
Things to do | With the demographic window narrowing, India must pursue simultaneous reforms across multiple fronts, treating this as an urgent national priority:
- As India transitions from youth-heavy to working-age-heavy demographics, dramatically improving education quality – particularly in govt schools – becomes essential. This must be paired with expanded vocational training and apprenticeship programmes, requiring close collaboration between govt, NGOs, and industry. India should rapidly adopt successful strategies from East Asian economies.
- Complexity of starting and operating businesses in India calls for business environment reform. Common complaints centre on excessive approval requirements and sluggish bureaucratic processes, compared with other nations. Multiple regulatory bodies and complicated tax, labour and other laws should be streamlined and aligned with global best practices.
- Labour-intensive manufacturing, particularly in the MSME sector, deserves stronger incentives, as do employment-heavy sectors like tourism and other services.
- Substantially increasing women’s labour force participation requires expanded skills training programmes, and improved child support infra. These can enable greater workforce engagement.
- Addressing infra bottlenecks is critical, including reliable non-fossil-fuel power supply, adequate water availability, robust transportation networks, and improved access to global supply chains.
- Regulatory simplification is equally important. Labour, tax, and regulatory obstacles that currently constrain services sector expansion, must be systematically removed.
The measures needed to capture India’s demographic dividend are largely well understood and documented. What remains lacking is genuine urgency – a recognition that transformational opportunities of this magnitude arise infrequently in a nation’s history. India owes it to both current citizens and future generations to seize what remains of this window before it closes.
Perhaps, most importantly, policymakers and citizens alike must internalise that a demographic dividend is not an entitlement guaranteed by favourable population statistics alone; it must be actively earned through sustained, coordinated effort across education, business environment, infra, and social policy.
Nigeria and Sri Lanka are sobering reminders that demographic opportunity, without decisive action, simply passes by, leaving behind unrealised potential and a harder path forward once the window shuts.
